Each new employee (faculty or staff) or student staff employee who needs network access will need to have an account request form submitted.

Instructions

Please fully complete the Technology Access Request form. A supervisor may submit this form well in advance of an employee’s first day, provided (s)he has all the required information for the new employee.


Frequently Asked Questions

When should I use this form?
 This form should be used for all new faculty or staff members to request network access. This form should also be used if an employee moves to a new department and needs access to new technology resources.

What should I do if I want to request a change in accounts or resources for an existing employee? 
 If an employee transfers into a new department, please resubmit the form as determined by the position's requirements. A form is not required for deleting accounts or minor changes of resources for currently employed faculty or staff members. To make these changes, contact the Help Desk at extension 6627 or email us at support@evergreen.edu.

Who should fill in this form?
 The new employee's supervisor should obtain all necessary information and submit the completed online form well in advance of the employees first day of work in order to allow for adequate processing time.

How long does it take to complete my request once it is submitted?
 New account requests can typically be processed in 2 or 3 business days. Please note, however, that special requests may take substantially longer to complete. For more information, contact the Help Desk at x6627.

How will I know that my account has been created OR how will I know that my employee's account has been created? The Help Desk will contact the supervisor via email once the account is ready. A supervisor may provide an alternate contact within the work area if they would like another individual to receive notification that the account has been created.
