Using Adobe Acrobat Pro: open PDF in AAP, go to Tools > Content > Edit Document Text
For a list of software one can use for this, please see PDF, Everything you ever wanted to know: Edit (Import)
For those using Word 2013 or 2016: go to File > Open > Find the PDF and click Open (formatting may need to be adjusted)

With Adobe Acrobat Professional
- Combine Files --> Add Files --> Preview --> Arrange Pages / Delete Pages --> Combine files
Using smallpdf.com - Free! Available for Mac and Windows!
With Microsoft Word
- INSERT --> Object --> Text from file
  - Can be any text file format {pdf, docx, otd, txt}
