Getting Started

Create a Form

Use the Evergreen Form Template

Because forms can collect sensitive data, including user email information, always use Evergreen-approved templates for college business to help users identify official college forms.

  1. TBD - Link for protected form template
  2. Click on the more options ... and select Copy to make a duplicate.
  3. Click on your duplicate and rename
  4. Please note: If you started creating form without using the official Evergreen template you won't be able to change the formatting to the official look. The workaround is to make a new form using the template and copy and paste your form questions and adjust any branching settings.


Forms from Lists

Microsoft Lists can also create forms to more easily collect list data. Create a Form from a Microsoft List
