Forms can be an easy way to gather responses as a poll, quiz, survey, and compile data into metrics or a spreadsheet in real time. You can share forms with a link, email invite, or a QR code.

Create a Form

Use the Evergreen Form Template

Forms can collect sensitive data with user email information. To protect the security of the content you are collecting, please only use Evergreen-approved templates for forms related to official college business to limit the possibility of a phishing scam.

1. TBD - Link for protected form template
2. Click on the more options ... and select Copy to make a duplicate.
3. Click on your duplicate and rename
4. Please note: If you started creating form without using the official Evergreen template you won't be able to change the formatting to the official look. The workaround is to make a new form using the template and copy and paste your form questions and adjust any branching settings.
