Create a Form

Using the Evergreen Template

Forms can collect sensitive data with user email information. To protect the security of the content you are collecting please only use Evergreen approved templates for forms related to official college business to limit the possibility of a phishing scam.

1. TBD - Link for protected form template
2. Click on the more options ... and select Copy to make a duplicate.
3. Click on your duplicate and rename
4. Please note: If you started creating form without using the official Evergreen template you won't be able to change the formatting to the official look. The workaround is to make a new form using the template and copy and paste your form questions.
